You can safely keep breast milk out of the refrigerator for up to 2 hours before it goes sour and should be thrown out.
If you've had the breast milk previously frozen and then thawed it out in the refrigerator then the breast milk can last up to 4 hours at room temperature.
It's best though to keep the breast milk refrigerated until ready to warm it up and feed the baby just to keep the milk from spoiling.
