If the case is to go on and the person who committed the crime is to be convicted then yes the victim in the case has to testify in court.
Without the victims testimonial there may not be enough evidence against the individual and then the case may be dismissed.
When you go to court to testify against someone who harmed you or did anything bad to you then there will be cops inside the court room for protection and likely the person in court who is being tried will be shackled if they were already in jail prior to the court date.
Also you may be legally summoned to court through papers served on you and if you fail to appear in court to testify you could be arrested through a warrant for failure to appear.
The best thing to do is to appear in court and testify no matter how hard it is.
