What happens if A victim refuses to testify in court?

In most cases when you're a victim of a crime and it goes to court you'll be served a subpoena and be ordered to court to testify in the case.

If you fail to show up at court to testify then the court and judge can put a warrant out for your arrest to make you appear in court.

Even though that is not right that's the way it is and if you get a subpoena to show up in court you need to show up and testify against the defendant.

Even if you don't show up to court the state can then charge the defendant and go on with the case and proceed with the trial if there is enough evidence.

There will usually be guards to protect you from the defendant especially if they have committed a violent crime against you.
