My 3 year old toddler boy refuses to use the toilet and he refuses to wear underwear. Whenever I put underwear on him he removes it and and throws a fit until I put his diaper back on him.

He throws a fit even when I start to remove his diaper to go potty.

How can I get my 3 year old son to give up his diapers and use the toilet instead?

If your 3 year old is not showing interest in wanting to use the toilet then I'd just back off for now and wait until he's 4 years old and try again.

If he doesn't potty train when he turns 4 then you should start forcing it on him.

Different kids potty train at different ages and 3 is still pretty young.

But if you really want to force him then you'll have to take his diaper off and then deal with some messes around the house for awhile until he realizes that he should be peeing and pooping in the toilet instead of having to clean up the messes on the floor.

If he does have an accident on the floor have him help you clean it up and tell him that if he used the toilet he won't have to clean up the messes.

But leave the diaper off of him no matter how much he screams. Although I'd just put the diaper back on and give up for awhile.
