A babies or toddlers pacifier should be replaced every 2 to 3 months for purposes of hygiene but other than that the pacifiers should be checked before each use for any cracks tears etc.
If the pacifier has any tears or cracks then it can pose a choking hazard and should be thrown out and replaced with a new pacifier.
