First a good way to prevent diaper rash is to regularly change your babies or toddlers diaper before it gets too wet or right after they poop.

A poop diaper should always be changed right after the baby or toddler gets done pooping and/or as soon as you notice it.

Poop can really cause a bad diaper rash.

For pee change the babies diaper every 2 to 3 hours and wipe the baby or toddlers diaper area using plain water or water and soap instead of baby wipes.

Always wipe the baby even when the baby has just peed since pee turns to ammonia and causes diaper rash.

You can get rid of diaper rash naturally by making a baking soda and water paste and apply the baking soda paste to your babies diaper area.

Or you can apply Desitin or Boudreaux's Butt paste for diaper rash as both of them are good for getting rid of and preventing diaper rash.

Whenever I have a heat rash I use Boudreaux's Butt Paste and it clears the heat rash up as well and my sister uses it on her son for diaper rash and swears by the Boudreaux's Butt Paste.
