You can tell if someone is mentally unhealthy by looking for signs and symptoms of a mental illness which are.

Decline in Functioning.
Chronic Irritability, Sadness, or Stress.
Changes in Eating Habits.
Sudden Disruption in Sleep.
Intense, Inconsistent Moods.
Social Withdrawal.

Feeling sad or down.
Confused thinking or reduced ability to concentrate.
Excessive fears or worries, or extreme feelings of guilt.
Extreme mood changes of highs and lows.
