You can't fire a doctor unless you're the doctors boss and they are employed by you.

However you are free to find another doctor and then let them know that you're no longer seeing that doctor.

You have the right to change doctors at any time that you want too.

The difference between a doctor and a specialist is that the doctor is just trained in basic medical things and can provide diagnoses and treatment for certain conditions.

A specialist is also a doctor but is trained more in a certain field of medical such as bone health, heart health etc and some specialists can also do surgery while regular doctors cannot do surgery.

A specialist can see a patient without a referral.

Although if you see a specialist without a referral then you have to pay for the cost of the specialist doctor visit out of pocket because health insurance won't pay for it unless a doctor refers you to a specialist.
