To stop your face from breaking out you should use facial cleanser to clean clogged pores and also do the following.

Properly wash your face.
Know your skin type.
Use a moisturizer.
Use over-the-counter acne treatments.
Stay hydrated.
Limit makeup.
Try not to touch your face.
Limit sun exposure.

You can also apply some facial creams that treat acne and other skin breakouts.

The signs that your acne is hormonal is acne that often reappears in the same spot because of oil production patterns.

If you're breakouts of acne occurs in the same area then it's most likely hormonal acne.
